The Wellington Primary Care Partnership is a voluntary partnership of 30 health and community-based agencies in the Wellington Shire.

Our goal is to improve the health and social wellbeing of our local community.

Our objectives are to:

• build and strengthen community partnerships
• work with our service providers such as GPs, community nurses, physios, counsellors, aged care, disability and home care workers to better coordinate their services
• improve access to community health services for consumers and carers
• be more responsive to community needs
• identify and focus planning around local and regional priority health issues
• encourage consumers to be active participants in their own health care by supporting individuals and families to better understand their health problems
• provide better preventative, rehabilitative, maintenance and support programs for people with complex health needs
• offer better health education activities that emphasise healthy behaviour and promote sustainable lifestyle choices
