The West Gippsland Healthcare Group Community Palliative
Care Service aims to care for and assist people facing
a life threatening illness, and provide their families
with the support necessary to care for their loved
ones at home.
Both the patient and carer(s) are actively involved
in care-planning, as it is the intention of our Community
Palliative Care Service to look at the patient’s
whole circumstances when providing the physical, emotional, spiritual, psychosocial, cultural & linguistic diverse care and support required.
The Service is provided by the District Nursing Service,
in liaison with a team of skilled health professionals
appropriate to the care requirements of each individual
patient. Trained volunteers are available to provide comfort
and support to patients and families.
The service is provided at
the home of the patient
or other agreed location, including a 24/7 on call
service, as discussed with patient and family.
The cost of the service and most of the equipment
is funded. Any relevant charges will be discussed
as required.

Philosophy, Aims and Purpose
The service provides a palliative approach for patients
and their families with the focus on pain management,
symptom control, bereavement risk assessment, counselling
and care co-ordination.

Patient Care
The palliative care service is provided to patients
in their home, in hospital or in another agreed location
Referral and Liaison with other services
Referral and liaison with other services and Health
professionals is made by the Palliative Care Team Leader,
the District Nurse or the GP after discussion with
patient and carer.
Education
Education regarding caring for the patient is ongoing
by the District Nursing staff and the patient and carer.
District Nursing staff regularly attend education sessions
to maintain and improve their palliative care skills
and knowledge.
