The West Gippsland Healthcare Group Community Palliative Care Service aims to care for and assist people facing a life threatening illness, and provide their families with the support necessary to care for their loved ones at home.

Both the patient and carer(s) are actively involved in care-planning, as it is the intention of our Community Palliative Care Service to look at the patient’s whole circumstances when providing the physical, emotional, spiritual, psychosocial, cultural & linguistic diverse care and support required.

The Service is provided by the District Nursing Service, in liaison with a team of skilled health professionals appropriate to the care requirements of each individual patient. Trained volunteers are available to provide comfort and support to patients and families.

The service is provided at the home of the patient or other agreed location, including a 24/7 on call service, as discussed with patient and family.

The cost of the service and most of the equipment is funded. Any relevant charges will be discussed as required.
