 S2S eReferral Agency Resource Person Training
S2S eReferral ‘Agency Resource Person’ Training sessions will be held across Gippsland during November/December with the intention of improving the capacity of agencies to be self-sufficient with regard to their basic S2S training and administrative tasks.
The sessions will focus on the essentials that an ARP must know to teach other staff the basic functions necessary to send, receive and process eReferrals and perform everyday S2S ‘housekeeping’ tasks for their agency such as registering staff and dealing with lost passwords.
The session for agencies based in Latrobe and Baw Baw will be held from 1.00-4.00pm on Wednesday 12 November in the Community Room, Latrobe Community Health Service, 42-44 Fowler Street, Moe.
The session for South Gippsland/Bass Coast agencies will be held from 10.00am-1.00pm on Wednesday 19 November at GPA South Gippsland, 11 Williams Street, Inverloch.

Introduction
The Better Health Care in Gippsland (BHCiG) project encompasses the development of a regional
approach to improving service coordination for consumers, with a particular focus on people at risk
of or experiencing chronic disease.
The main project components are:
- Partnerships - The Gippsland Heath Services Partnership has now been established with
membership from the Acute, Community Health, General Practice and Local Government HACC
service providers throughout Gippsland.
- Service Coordination - the development and implementation of an agreed regional service
coordination model, which is inclusive of General Practitioners and Hospitals. This model will
facilitate information flows between acute health and primary care services.
- Chronic Disease Management and Care Pathway Protocol - the development and implementation
of a regional model of care for people with chronic disease and a comprehensive care pathway
protocol for a specific condition.
- Evaluation - an external evaluator will be engaged to monitor and evaluate the implementation
of these project components.
The project is aimed at implementing sustainable changes to the way in which services are
coordinated and delivered in an integrated way. The work program is directed toward service
system change and the implementation of common, agreed ways of doing business, rather than
additional or new services.
