 The Jumbos: Gippsland eReferrer of the Year Award
It’s been another very exciting year for S2S eReferral in the Gippsland region. New agencies have come on board with S2S and the system has also expanded to many more sites, services and staff at existing agencies. From a total of 535 eReferrals sent in June 2007, we reached a whopping 1946 referrals sent across the region in June this year - even cracking the 2000 mark for referrals both sent and received for the first time in May. With statistics like that, it’s hardly a surprise that Gippsland has led the way across the state during the year – now consistently sending more eReferrals each month than the rest of Victoria put together!
Against that background, it has proved extremely difficult to pick the winners for the 2007-08 ‘Jumbos’. There have, however, been a few people who have stood out as true eReferral ‘champions’ in the Gippsland region.

Introduction
The Better Health Care in Gippsland (BHCiG) project encompasses the development of a regional
approach to improving service coordination for consumers, with a particular focus on people at risk
of or experiencing chronic disease.
The main project components are:
- Partnerships - The Gippsland Heath Services Partnership has now been established with
membership from the Acute, Community Health, General Practice and Local Government HACC
service providers throughout Gippsland.
- Service Coordination - the development and implementation of an agreed regional service
coordination model, which is inclusive of General Practitioners and Hospitals. This model will
facilitate information flows between acute health and primary care services.
- Chronic Disease Management and Care Pathway Protocol - the development and implementation
of a regional model of care for people with chronic disease and a comprehensive care pathway
protocol for a specific condition.
- Evaluation - an external evaluator will be engaged to monitor and evaluate the implementation
of these project components.
The project is aimed at implementing sustainable changes to the way in which services are
coordinated and delivered in an integrated way. The work program is directed toward service
system change and the implementation of common, agreed ways of doing business, rather than
additional or new services.
